Applying on The Marketplace

  • With a free Indiana Navigator:
    Indiana Navigators are trained and certified to help you apply and enroll in a Marketplace plan. Learn more about Navigators and find one near you.
  • Online:
    If you’re not interested in free Navigator help, you can visit HealthCare.gov to apply on your own. Click here to get ready. Open Enrollment is November 1 through December 15, 2017, but you may be eligible for a Special Enrollment Period (SEP).
  • Over the phone:
    Call the Marketplace Call Center 24/7 at
    1-800-318-2596.
  • Paper application:
    Download a copy of the application and mail your completed copy to:

APPLYING FOR MEDICAID (INDIANA HEALTH COVERAGE PROGRAMS)

There are four ways to apply:

    • With a free Indiana Navigator:
      Indiana Navigators are trained and certified to help you apply and enroll in a Medicaid plan. Learn more about Navigators and find one near you.
    • Online:
      Visit here and select Apply for Health Coverage, SNAP, and/or Cash Assistance online. If you’re unsure if you qualify for Medicaid, you can also apply and shop on the Marketplace. The Marketplace will transfer your application back to the Medicaid office if they think you’re eligible for an Indiana Health Coverage Program, like HIP.
    • Over the phone:
      Call 1-800-403-0864 between the hours of 8:00 a.m. and 4:30 p.m., and a staff member of Indiana’s Division of Family Resources (DFR) will assist you with your application.
    • Paper application:
      Access and print your application, and mail your completed application to:
